All travelers must possess a Taiwan national health examination certificate and a Taiwan national health declaration form. Children under 18 years old are not eligible for either.

The Taiwan Health Declaration Form is probably among several forms that are readily available for people in Taiwan who need to seek the help of a qualified medical professional. It is worth noting that the Taiwan health statement form includes a list of contagious disease, including influenza, measles and cholera. Anyone who has any of these signs must be quarantined.

In addition, the Taiwan health declaration form requires all travelers to declare any prior history of travel to tropical or subtropical countries and to provide the necessary immunizations. Additionally, Taiwan requires travelers to submit their passports when entering the country. In some instances, airlines and hotels may also require this document. For a period of one year from the date of its signing, from either the date of departure or arrival in Taiwan, an immigrant may not leave the country without obtaining both the Taiwan national health examination certificate and the Taiwan national health declaration form.

It is easy to obtain the Taiwan health declaration form. You need to have a computer with internet connection and access to the web. You have the option of registering for either a 1-year or 2-year period. The one-year registration allows for more flexibility and allows you to use the service for a longer time (1 year) from the date it was signed. The two-year registration, on the other hand is more costly and allows you to use the service for a shorter time of two years after its signing.
